We require a fully qualified Social Worker with Hospital dicharge experience.
*Must be a qualified Social worker and registered to SWE.

Full enhanced Adult DBS a requirement.
Full clean driver’s licence and access to a car (or able to use another appropriate means of travel) to travel to a range of locations including home visits.
