Assessment and care management
KEY DUTIES AND RESPONSIBILITIES
Carry out basic need based assessments with clients and/or carers within agreed procedures, to determine specific individual client’s needs, level of risk and urgency of response so that an individual care programme can be identified.
Implement, co-ordinate and maintain individual care plans and act as key worker in appropriate cases to ensure that services are arranged to meet identified and appropriate needs.
As part of a social work team, monitor and implementation of individual care plans, referring significant changes in needs to appropriate parties to ensure that through the review process the care plan is changed to meet these needs.
Liaise and ensure an effective dialogue with other service provides/carers e.g. social work staff, other community service providers, relatives/carers etc, to ensure their involvement in the care process so that the full range of services is available to meet the client’s need.
Maintain client records/systems to ensure the provision of accurate, up to date client related information to the group.
As a member of the team contribute to the development of services by contributing views and ideas to working groups to ensure the continuing provision of a high standard of care to clients.
