A great opportunity to join a friendly team of experienced LD Nurses, working in the London Borough of Sutton. As an LD Nurse you will be working on a varied caseload from Continuing Health Care checklists and reviews, work with Supported Living homes and individuals in their own homes. Working to ensure all our clients have their health needs met and complete their health screening, you will be working closely with GP’s and PCN’s. There are also opportunities to work with the varied Multidiscipline team members in the Clinical Health Team for Adults with a Learning Disability.
