MAIN PURPOSE:

  1. To assess the needs of people with acute, complex or enduring mental health issues and the needs of carers.
  2. Together with the service users and carers, formulate and implement care plans appropriate to their needs.
  3. To monitor, review and evaluate services in order to ensure that they are provided effectively in accordance with need.
  4. Within the setting of a multi-disciplinary Rehabilitation Support Team, provide rehabilitation packages of care to service users.
    The RST Team offers an extended hour’s service to individuals and the Team work to a Flexible Hours contract.

SUMMARY OF RESPONSIBILITIES AND DUTIES:

  1. Provide an assessment and care management service to individuals, their carers and other agencies.
  2. Ensure the provision of an assessment, care co-ordination and care management service under CPA for service users and carers, and in collaboration with professional colleagues and representatives of voluntary and private agencies, and where appropriate, to undertake the role of Care co-ordinator.
  3. Monitor, review and evaluate care plans in order to ensure that they are meeting the needs of service users.
  4. Hold a small caseload concentrating on those with complex care needs.
  5. Review the needs of individual service users and their carers and to undertake Carer’s Assessments where appropriate.
  6. Identify and report unmet needs and short-falls in the construction of individual care plans.
  7. Maintain client records in accordance with Oxleas NHS / Bromley Council Policy.
  8. Respond to and where possible resolve complaints within the Oxleas NHS / Bromley Council Complains procedure.
  9. Contribute to service development by providing information and ideas for future services.
  10. Advise manager on quality and value for money of purchased services.
  11. Co-ordinate, participate and where appropriate chair case conferences / plans meetings.
  12. Participate in multi-disciplinary and inter-agency meetings and working groups and represent the service as required.
  13. After training, perform the duties of an Approved Mental Health Professional (formerly ASW).
  14. Undertake any other duties commensurate with the level of the post, as required to ensure the efficient and effective running to the Department/Section.

KNOWLEDGE

  3. Broad knowledge of local mental health resources in both statutory and voluntary sectors.
  4. Knowledge of NHS and Community Care act and its requirements on the Local Authority
  5. Knowledge of Carers Act and its requirements on the Local Authority.
  6. Knowledge of the role of the Approved Social Worker.

EXPERIENCE

  1. Experience of working with people with complex, acute and/or enduring mental health needs which may be challenging at times.
  2. Experience of working in collaboration with other mental health workers, both in the community and hospital setting.

QUALIFICATIONS
BA SW /DIPSW /CQSW
Registration with the General Social Care Council (GSCC) is mandatory.

The pay rate is £17.75 per hour

LBN is seeking to recruit 5 staff on a temporary basis (initially 2-3 months, subject to possible extension), to support the delivery of essential grant administration work, to local businesses who have been negatively impacted by the Covid pandemic.
The role is working on a full time basis (36 hours a week), with a usual working pattern of 9am – 5pm (there is some flexibility to adjust this if required e.g. 8am – 4pm or 10pm – 6pm).

Some examples of the duties include;
• Providing recommendations against each grant application.

In addition to the skills, knowledge and experience set out, candidates must also have experience of supporting SEND services and should have experience of using a range of Microsoft tools including Sharepoint lists, PowerBI, PowerAutomate.
Knowledge
An expert understanding of the national and local context for and operations of children’s services, including education, children’s social care, and safeguarding.

An expert understanding of statutory, policy and regulatory frameworks with regard to children’s services.

Advanced analytical and numerical skills and a demonstrable ability to work with complex datasets.

Qualifications & Experience

Relevant professional qualification or equivalent experience of children’s services.

Experience of managing performance management, intelligence and data quality arrangements and operations in a high performing team.

Ability to produce information for external regulators including for inspection.

Experience of data protection and safeguarding frameworks.

Ability to gain expertise in delivering performance information and high quality intelligence and analysis products to support improvement.

Experience of working within a matrix management environment to contribute to departmental and organisational outcomes.

Ability to analyse a range of data, including the use of statistical
techniques, to support evidence based decision making for commissioning, service design and strategy development.

Deploy a range of problem solving skills with a proven ability to achieve results.

Interpret complex guidance and legislation and convey requirements in an easy and understandable format.

Ability to provide an expert lead in two or more of the following areas:
GIS mapping
Data visualisation techniques
Data linkage and processing
Advanced statistical techniques
BI tools (preferably PowerBI)
Data extraction and analysis using SQL and R.

Experience of supporting the development of the JSNA process, working closely with staff across the council, CCG, CSU and partnership.

Experience of using a range of software and systems relevant to children’s services in support of data extraction, manipulation, analysis, presentation and submission, for example: Microsoft Office applications (e.g. Excel, Access); Business Objects Infoview & Web Intelligence, ARCView GIS, Frameworki/Mosaic, DFE COLLECT portal, Synergy, Crystal reports (SQL), Covalent/Pentana

MAIN PURPOSE OF THE JOB

  1. Improve outcomes for children and young people through applied educational psychology.
  2. Provide a range of psychological services for children, young people and their families, and schools.
  3. Develop innovative and inclusive services.

DUTIES & RESPONSIBILITIES

  1. Deliver EP services which promote person centred planning and involving children and young people in decision making
  2. Promote applied educational psychology practice in Tower Hamlets
  3. Work with schools and settings to identify and meet the needs of children and young people
  4. Provide psychological advice for Education, Health and Care needs assessments and psychological input for children and young people with EHC plans
  5. Promote positive emotional health and wellbeing for children and young people through therapeutic interventions, professional training, supervision, and staff and parent support
  6. Provide concise written reports of involvement
  7. Provide advice to other LA officers and professionals on matters relating to the educational needs and psychological development of children and young people
  8. Promote and comply with the Council’s equality policies
