MAIN PURPOSE:
- To assess the needs of people with acute, complex or enduring mental health issues and the needs of carers.
- Together with the service users and carers, formulate and implement care plans appropriate to their needs.
- To monitor, review and evaluate services in order to ensure that they are provided effectively in accordance with need.
- Within the setting of a multi-disciplinary Rehabilitation Support Team, provide rehabilitation packages of care to service users.
The RST Team offers an extended hour’s service to individuals and the Team work to a Flexible Hours contract.
SUMMARY OF RESPONSIBILITIES AND DUTIES:
- Provide an assessment and care management service to individuals, their carers and other agencies.
- Ensure the provision of an assessment, care co-ordination and care management service under CPA for service users and carers, and in collaboration with professional colleagues and representatives of voluntary and private agencies, and where appropriate, to undertake the role of Care co-ordinator.
- Monitor, review and evaluate care plans in order to ensure that they are meeting the needs of service users.
- Hold a small caseload concentrating on those with complex care needs.
- Review the needs of individual service users and their carers and to undertake Carer’s Assessments where appropriate.
- Identify and report unmet needs and short-falls in the construction of individual care plans.
- Maintain client records in accordance with Oxleas NHS / Bromley Council Policy.
- Respond to and where possible resolve complaints within the Oxleas NHS / Bromley Council Complains procedure.
- Contribute to service development by providing information and ideas for future services.
- Advise manager on quality and value for money of purchased services.
- Co-ordinate, participate and where appropriate chair case conferences / plans meetings.
- Participate in multi-disciplinary and inter-agency meetings and working groups and represent the service as required.
- After training, perform the duties of an Approved Mental Health Professional (formerly ASW).
- Undertake any other duties commensurate with the level of the post, as required to ensure the efficient and effective running to the Department/Section.

EXPERIENCE
- Experience of working with people with complex, acute and/or enduring mental health needs which may be challenging at times.

QUALIFICATIONS
BA SW /DIPSW /CQSW
Registration with the General Social Care Council (GSCC) is mandatory.

LBN is seeking to recruit 5 staff on a temporary basis (initially 2-3 months, subject to possible extension), to support the delivery of essential grant administration work, to local businesses who have been negatively impacted by the Covid pandemic.
The role is working on a full time basis (36 hours a week), with a usual working pattern of 9am – 5pm (there is some flexibility to adjust this if required e.g. 8am – 4pm or 10pm – 6pm).
Under current national government guidance, these roles will be based working from home. ICT equipment will be provided on loan for the role (this will need to be collected by the successful candidate in person at our Newham Dockside offices). At a later date, there may be a requirement to attend our office, which is based in Stratford, E20.
The role is based in the Our Newham Business and Enterprise Team.
The role is responsible for assessing additional restrictions business grant applications within agreed timescales and in line with the organisation auditing principles.
Some examples of the duties include;
• Providing recommendations against each grant application.
• Providing inputs to ongoing planning with grant making team
• Ensuring that grant assessments are completed according to the operational and financial needs of the programme in a time-pressured environment.
• Keeping the relevant internal stakeholders informed about progress against upcoming deadlines and deliverables.

MAIN PURPOSE OF THE JOB
- Improve outcomes for children and young people through applied educational psychology.
- Provide a range of psychological services for children, young people and their families, and schools.
- Develop innovative and inclusive services.
DUTIES & RESPONSIBILITIES
- Deliver EP services which promote person centred planning and involving children and young people in decision making
- Promote applied educational psychology practice in Tower Hamlets
- Work with schools and settings to identify and meet the needs of children and young people
- Provide psychological advice for Education, Health and Care needs assessments and psychological input for children and young people with EHC plans
- Promote positive emotional health and wellbeing for children and young people through therapeutic interventions, professional training, supervision, and staff and parent support
- Provide concise written reports of involvement
- Provide advice to other LA officers and professionals on matters relating to the educational needs and psychological development of children and young people
- Promote and comply with the Council’s equality policies
