Job Purpose:   

The Social Work Assistant in the Community Mental Health Team (CMHT) for Older People will support the Community Mental Health Team in providing comprehensive mental health services to older adults. This role is dedicated to enhancing the well-being and recovery of citizens utilising strengths-based, person-centred approaches.

The role involves evaluating and creating individualised care plans based on a person’s strengths and resources. It includes managing a case load and offering direct support to older adults during their recovery, encouraging independence and resilience. 

You will work in collaboration with multidisciplinary team members to implement strategies that facilitate mental health recovery. You will be working in partnership with people to identify their outcomes and work with social workers to implement care and treatment plans to support their identified outcomes.






Job Specific Requirements

1.To work in partnership with individuals to help them identify their strengths, needs and outcomes.

2.To support individuals to maximise their independence and develop coping skills.

3.To undertake assessments including wellbeing assessments and risk assessments with individual’s and their networks, whilst considering outcomes, available resources, policy and legislation.

4.To make recommendations regarding care and treatment plans to support individuals to meet their identified outcomes, considering the flexible use of all resources and the individuals support network to meet identified needs.

5.To work alongside social workers to implement, monitor and review care and support packages / care and treatment plans.



Education 

& Training

            GCSE’s including mathematics and English.

Post GCSE qualification.

Experience / Knowledge

            Experience of working with adults or people with care and support needs. 

An understanding of the issues that impact upon people who have a Mental Health condition.

Skills and

Abilities

            Skills-

Well developed literacy and numeracy skills and the ability to write clear reports.

Skilled at gathering and collating information.

Abilities-

Ability to communicate effectively with adults with care and support needs.

Ability to develop good working relationships with individuals, families, and other professionals.

Ability to advocate and liaise with other professionals on behalf of individuals and their families.

Ability to act on your own initiative and also as part of a team.

Ability to organise your own workload to meet priorities, time constraints and deadlines.

Ability to engage in supervision and use it appropriately.
