Outreach Team
Purpose of the Role:
To provide care and support to adults living with dementia, learning disabilities, autistic spectrum disorder, older people and people with a sensory or physical impairment, either in the community or home based. The focus being to support individuals to regain confidence, maximise independence and to continue living at home and access the community. The service provides Long- or short-term support
We must ensure those we support and their informal support network (if they have one) is at the centre of all decisions about how they are supported. You will provide the service in such a way that the adult is able to feel secure, confident and included in all decisions regarding the service provided to them.
Services must be delivered in line with CQC standards.
Examples of services that may be provided:

⦁ travel training
⦁ support to access education or training
⦁ support to access social activities/ community
⦁ support to access and maintain employment
⦁ support to access benefits and finance management
support to access medical support to maximise health and wellbeing
Skills, Experience, Knowledge, Behaviours
Essential:
- As a regular and intrinsic part of this role requires you to speak to members of the public in English, the ability to converse at ease with customers and provide advice in accurate spoken English is essential and consistent with the requirements of this role. This role also requires you to be polite and courteous when conversing with the public.
- A Social Care qualification i.e. NVQ/QCF 3 in Social Care or equivalent.
- Competent when using the IT systems and ability to maintain electronic records in accordance with statutory requirements and data protection policy.
- An understanding of domiciliary care services regulatory requirements
- Minimum of 2 years’ Experience of supporting vulnerable adults with home and community based personal care and support.
- Ability to work independently using own initiative and as part of a team while managing workload effectively, prioritising work and adopting a flexible can-do approach to changing demands of the service.
