Enhanced DBS

The applicant will be based in the Hospital Social Work Team, based in King George Hospital. The team had an office based in Whipps Cross hospital. The team is responsible for implementing the discharge to assess model of care for patients who are deemed medically fit or completed their rehabilitation. It manages referral from out of borough hospitals, work with the Hubs to agree discharge pathways and plans. The team deals with safeguarding referrals, complete Care Act assessment, provide advice and information regarding all social problems

Qualifications/Certifications
HCPC Registration
Degree level or equivalent e.g. NVQ level 4 &/or relevant experience
GCSE Grade A-C (or equivalent) in English Language and Mathematics
