We are looking for an experienced SW with an extensive knowledge of the NHS Continuing Health Care Framework 2018.
The candidate is able to complete CHC Checklists, (London Health Needs Assessments) and has experience in progressing cases through to the DST stage.
The candidate will be able to understand what a Primary Health Care need is and where Social Care can no longer legally provide services and support to service users.
The candidate will be working within the Integrated Health and Social Care Service called the Enfield Integrated Learning Disabilities Service (EILDS) and will carrie the ethos of joint working with Enfield CCG.
The post with be available for 3 – 6 months and we are expecting the person to have a short induction and then immediately start to progress those cases we believe meets the CHC eligibility threshold for full CHC funding.
