Brief summary of the role and responsibilities:

• Minimum standard of English GCSE Grade A -C or equivalent.

SPECIAL REQUIREMENTS:
• Car Driver must be mobile and able to travel independently to various locations around the borough carrying equipment.

We are looking for an experienced Care Manager Assistant to work closely with the Senior Social Worker to avoid Social Admissions to Hospital. This will involve supporting Carers by completing urgent assessments and support plans to prevent Cares breakdown. Also supporting Vulnerable adults ensuring they have the appropriate support to meet their assessed needs.

You will have a knowledge/experience of facilitating safe Hospital discharges, strong working knowledge of the Care Act 2014 and its application. Timely strength-based assessments Also, knowledge of the Mental Capacity Act and experience completing Mental Capacity assessments.

You will be supporting Younger Adults with physical disabilities and sensory impairment and Older Adults to live as independently as they are able and work with their carers to ensure risks are appropriately managed. We are seeking a dynamic and experienced interim Strategic (Lead) Commissioning Manager to lead older adult commissioning teams within the adults commissioning service until a permanent member of staff is recruited.

Social Worker Mental Health required for 22 hours per week. Based in North Powys, flexible working available. AMHP desirable. Pay rate approx £35 per hour – negotiable depending on experience and qualifications.

Main Purpose of Post:
• To assess and manage the complex needs of people with a mental health issue and their carers to ensure their health, wellbeing and promote social inclusion.
• To liaise with professionals to ensure seamless care pathways which safeguard and promote independence.
• To promote joint working which supports the person centred outcomes to be delivered for people with a mental health issue.
• To undertake safeguarding investigations as required.
• To assist the Local Authority to meet their statutory obligations under Mental Health Measure and the Social Services and Wellbeing (Wales) Act 2014 and others.
• Undertake the role of an AMHP to deliver on the Local Authorities statutory requirements within the MHA 1983.

Experience:
• 12 months post qualifying  experience of working with people with mental health issues and their carers
• Experience of undertaking assessments of need and identifying and managing risks.
• Experience of complex casework 
• Experience of care management across the range of health and social care settings

Knowledge:
• Understanding of an individuals mental health needs and their carers needs.
• Understanding of safeguarding and the issues affecting people with mental health needs and their carers
• Knowledge of current legislation, national guidance and best practice relevant to the post.
• Knowledge and implementation of the Mental Capacity Act
 
Qualifications & Training:
• Appropriate social work qualification (i.e. Degree in Social Work, DipSW or equivalent).
• AMHP or willingness to undertake training at the first opportunity following appointment to post.
• Registration as Social Worker with CCW/CQC

Other requirements:
• Current driving licence or ability to travel extensively around the county.
