Join us – make a difference in our communities We’re on a drive to achieve our ambition to provide West Sussex residents with the tools and support to help children, young people and adults with Lifelong Disabilities to achieve the outcomes that are right for them. We are creating a Lifelong Service for anyone that has a lifelong disability – learning disability, physical or sensory disability, acquired brain injury or autism – acquired before the age of 25, providing a consistent approach where we understand and work in partnership with children, young people and adults with lifelong disabilities, autism and their families in order to: • Offer early intervention
• Build on an individual’s strengths to develop skills and independence
• Build and promote links with the individual’s community
• Promote and maintain safety and good health, and
• Facilitate employment. About the job As a Social Worker within a dedicated reviewing team, your role will involve working with adults with lifelong disabilities, including physical and learning disability and autism. You will be responsible for supporting the local authority deliver its Adult Social Care Strategy by undertaking quality, strengths-based Care Act Reviews. You will be required to work at pace, and to consider a whole person approach to reviews, not just the services a person receives, but one that recognises peoples’ strengths, taking into account the wider support network and assets. Workers will be encouraged to explore creative approaches to managing personal budgets, that do not rely on funded services. Reviews will be targeted where the authority has determined the best opportunities lie to deliver alternative means of support and achieve savings. You will be expected to work within organisational, policy and legislative requirements. Reviews will need to be in person where required, although the authority is adopting new ways of working which means SWs will not be required to be on site every day. It is expected however SWs will be required to spend approximately (and a minimum of) 2 days per week present in West Sussex (either completing face to face visits or being on site), the other 3 days can be worked at home. We would be happy to see applications for SWs who can work a minimum of 3 days a week
