Salary : £14.21 per hour.
We are looking for between 10 to 15 Wellbeing and Independent Practitioners to join our team in West to help provide support to the current service.
Candidate must have;
• Good computer knowledge – this role is currently working remotely
• Care knowledge
• Willingness to learn
• Good communication skills
• Admin experience – confident in Work, Excel and using corporate systems
• Ideally NVQ Level 4 in Care or Social Care but not essential
The pay rate for this position is £14.21 per hour and the team are looking to conduct interviews on 15th and 16th February with a view to start week commencing 22nd February.
Job Purpose
ECC Adult Social Care (ASC), through effective practice, is moving towards a transformational shift from a focus on long-term care support, to a model with four key principles: prevention; early intervention; enablement and safeguarding. This will enable people, their families and carers to access information and tools, live healthily and independently and for ASC, an opportunity to respond pro-actively to growing service demand in a sustainable way.
Working with people with disabilities and other conditions, you will undertake statutory responsibilities and focus on prevention, strengths, reducing risks and meeting needs. You will provide advice, working alongside people and their networks to support them to maximise their independence and wellbeing.
In collaboration with colleagues and practitioners across the health and voluntary sector, you will support effective integration between health and social care, ensuring systems are joined up and accessible.
