Job Description:
In this challenging but rewarding role, you will work as part of a strong and dynamic social work service. You will be required to act as an allocated social worker, responsible for delivering a statutory social work service to those people who access or need specialist mental health services and will support their discharge from hospital.
You will work in partnership with the newly formed mental health hospital discharge hub, including completion of all statutory work such as Care Act assessments, s117 aftercare planning meetings, safeguarding, Mental Capacity Assessments, Best Interest Meetings as well as any other social care related tasks and functions.
This will be a fast-paced role, and you will be required to be proactive and able to work under pressure to ensure all aspects of the individual’s needs are considered and incorporated into the discharge process. You will work as part of a small county-wide team which will focus solely on the discharge role. The workers within this team will be aligned to geographically based mental health hospitals, providing a service to people who are aged 18 and over.
The staff within the team may focus on a specific age group either 18-65 or 65+, but you will need to be willing to cover work across the age range. You will manage a caseload, collaborating with people and family or friend carers, undertake strengths-based assessments and reviews, identifying eligible needs and personalised outcomes.
You will act as a professional representative for the service by working with a focus on ensuring the safe and effective delivery of services to people who have complex mental disorder, acting with integrity, professionalism, and respect on behalf of the organisation, ensuring at all times that the needs of individuals and the business are paramount.
You will work effectively in partnership with other staff, teams and agencies, both internal to the Council and external, including Sussex Partnership NHS Foundation Trust. An excellent understanding of the relevant health, voluntary and private services in the area is essential. You will remain up-to-date and compliant with all relevant legislation and adhere to organisational procedures, policies, and codes of conduct as well as Social Work England professional standards to uphold a high-quality, seamless service.
You will be working within a supportive environment to apply your social work skills to develop and implement support plans, aimed at improving wellbeing and delivering outcomes, including utilising community assets, and ensuring that eligible needs are met. You will be working within a managed and supportive context, applying learning to further develop capability, and professional confidence.
You will be supported with professional supervision and assessment and a workload that is appropriate to your level of professional development. There will also be a requirement for you to work flexibly using your own initiative where appropriate to support people, carers, and families, sometimes out of normal working hours.
You’ll need to be able to travel independently throughout the county, including to areas that may not be easily accessible by public transport, so a driving licence is preferred. There is a regular and intrinsic requirement to communicate in English with members of the public.

Qualifications and/or experience:
• Degree in Social Work or a recognised equivalent Social Work qualification,
e.g. Diploma in Social Work.
• Registered with the relevant professional registration body and responsibility
for maintaining registration with that body.
• Approved Mental Health Practitioner (it is required to have AMHP
qualification, or to be working towards the qualification to be complete within
2 years of appointment and to maintain that knowledge).
• Experience of undertaking assessments and social work intervention
planning, implementation and review by means of creative and pragmatic
solutions.
• Experience of working in partnership with those who access the service and
maintaining a focus on the needs and strengths of the people, and the
outcomes they want to achieve, whilst handling conflict and disagreement.

Job Context:

A Senior Paediatric Occupational Therapist is required to cover the remaining six months of a maternity leave position.
Manage a caseload of major equipment and adaptation cases of varying complexity
and diversity for disabled children and young people within a framework of
supervision, including assessments and recommendations, evaluation and
approvals, preparation of reports, to effectively meet the needs of the client group
within the resources available and in line with Directorate and Corporate policy,
strategies and standards.
• Provide professional advice and guidance to other staff in the Directorate and multiagency staff on aspects of disability to enhance the decision-making process for the
appropriate provision of services within the resources available.
• Initiate and develop close joint working with statutory agencies, local housing
providers, District Councils, Home Improvement Agencies, Health Commissioners
and Provider Trusts, voluntary and private agencies and the independent sector to
facilitate an effective service delivery
• Ensure that information systems and records are effectively maintained to provide
reliable, up to date and accurate information as required by the Team Manager to
assist the process upon which decisions affecting service delivery can be made and
for auditing purposes to ensure adherence to Directorate policy and financial
regulations.
• Contribute to the development of new and existing practices and procedures through
attendance on Working Groups, training courses etc to develop effective working
practices that reflect government initiatives, guidance and legislation.

Experienced, skilled CPA required for a contract of around 9 months
ROLE PURPOSE:
This role integrates the responsibilities of safeguarding children at risk of harm with independent oversight of care planning for children in care. The postholder will ensure that robust safeguarding practices are in place, statutory responsibilities are met, and that services are delivered effectively to improve outcomes for children and families.
A key focus of the role is ensuring that children’s voices are central to all decision-making. The postholder will also chair Child Protection Conferences and Looked After Reviews in line with current legislation and the IRO Handbook, promoting active participation from children and young people while providing quality assurance for care planning and safeguarding processes
Enhanced Disclosure and Barring Service (DBS) clearance.
QUALIFICATIONS
Degree in Social Work or equivalent.
Registration with Social Work England.
Evidence of continuous professional.

Diploma/Certificate in management
EXPERIENCE
Significant experience in child protection, safeguarding, and care planning.
Proven ability to chair complex multi-agency meetings, including child protection conferences and care reviews.

Strong knowledge of relevant legislation, statutory guidance, and best practices in safeguarding and children in care.
Experience of supervisory and front-line management
