The person will:

The Child Health & Disability Team are seeking to recruit a 2 dynamic and motivated individual sfor the position of Support Planner. If successful, you will work alongside a well-established and dedicated Support Planning team whilst being based within Cardiff’s Child Health and Disability Team. The role will consist of providing support planning for Children and Young People with complex needs.
We are looking to recruit 2 candidates who are passionate about working alongside children, young people, their families and communities to bring about change and promote positive outcomes for each individual. They will need to have a strong value base, previous experience with working with children and young people with complex needs, strong organisational and time management skills, and have a commitment to a strength based practise and person centred initiatives.
You will work as part of a team to:
• Design high quality, person centred plans.
• Work to a clear value base that promotes self-determination and social inclusion within the child’s local community and support the child to establish and maintain community connections.
• Support and empower children who use our services to reach their full potential.
• Signpost and provide accessible information on a wide range of opportunities available.
• Liaise efficiently with a wide range of internal and external partners.
• Establish and maintain a wide range of community based integrated opportunities for Children.
• Monitor and review referrals to ensure they remain person centred and that agreed outcomes are met.
• Maintain appropriate records and compile reports in accordance with any format required.
• Although not essential, the team would welcome Welsh speaking applicants.
The position will be full time over a 37 hr working week Monday to Friday but may require occasional flexibility around core hours.

Cardiff Council are looking to recruit 2 experienced Social Workers to join our new and innovative team that will be independently reviewing Care and Support Plans for children and young people in Cardiff.
These are similar to the CIN plans in England
You will be joining an enhanced and revised Reviewing Hub, sitting alongside experienced Child Protection Conference Chairs and Independent Reviewing Officers. The focus and ethos of the Care and Support Plan Reviewing Officers, will be to ensure children and young people have the best plans that promote their wellbeing and safety.
Cardiff is committed to working in a strength based approach, utilising Signs of Safety. Knowledge and experience of this approach is essential, however training will be provided.
The Care and Support Plan Reviewing Officer role will introduce independent reviewing to all children and young people subject to a Care and Support Plan. You will need to review and make recommendations utilising outcome based care planning approaches, which will impact and improve lives of children and young people. This will include knowing and recognising where there are safeguarding concerns, or alternatively when non statutory services might better support a child or their family.
The main duties of the post holder will be to ensure you are focussed on the needs of children and ensuring they are addressed, minimising drift and checking the consistency of care planning and decision-making.
You should be a registered experienced Social Worker, have experience of working with Care and Support plans as well as Child Protection. You should also be able to demonstrate skills in chairing and reviewing care plans. An ability to work under pressure and to strict deadlines and timescales is important. Good communication and organisational skills are also vital for this role.
Education & Training
Degree in Social Work or equivalent.
Registered with Social Care Wales.
Experience / Knowledge
Significant Post qualifying experience in Child Care Social Work, including work in a statutory child care agency.
Direct experience of working with children on the Child Protection Register, Looked After Children, their parents and carers.
Experience in Social Work Supervision and Support.
Chairing of child care meetings.
Knowledge of Social Services and Wellbeing
Act, Wales Safeguarding Procedures and subsequent
legislation relating to
children.
Detailed knowledge of related guidance and regulations with particular reference to sexual exploitation, child trafficking, missing children, domestic abuse protocols, neglect, fabricated illness, forced marriage, female genital mutilation and resolution of professional differences.

Working in line with our statement of purpose, you will take responsibility for the smooth and safe running of the establishment, ensuring that young people receive a sensitive service best meeting their social, racial, psychological, educational, cultural, physical identity and health care needs.
You will be assuming key worker responsibilities by implementing, monitoring and recording individual personal plans in consultation with your supervisor and other members of the team. Working in a positive and respectful manner, you will provide individualised care through the development of professional and effective relationships.
You will have a knowledge and understanding of the basic needs of a young person and their rights. You will be able to demonstrate an ability to communicate and work individually and as part of a team with young people, their families, colleagues, other professionals and external agencies.
You will understand the importance and your responsibility of the safeguarding and protection of children, and how to appropriately escalate concerns if they are raised.
This is a physically, actively and emotionally challenging role which requires you to be able to meet those challenges.
The role will need you to have the ability to work within a shift pattern across the full week of earlies, lates, weekends and bank holiday working. May also be required to include sleep–in –duties.
