Cardiff Council are looking to recruit 2 experienced Social Workers to join our new and innovative team that will be independently reviewing Care and Support Plans for children and young people in Cardiff.

You will be joining an enhanced and revised Reviewing Hub, sitting alongside experienced Child Protection Conference Chairs and Independent Reviewing Officers. The focus and ethos of the Care and Support Plan Reviewing Officers, will be to ensure children and young people have the best plans that promote their wellbeing and safety.
Cardiff is committed to working in a strength based approach, utilising Signs of Safety. Knowledge and experience of this approach is essential, however training will be provided.
The Care and Support Plan Reviewing Officer role will introduce independent reviewing to all children and young people subject to a Care and Support Plan. You will need to review and make recommendations utilising outcome based care planning approaches, which will impact and improve lives of children and young people. This will include knowing and recognising where there are safeguarding concerns, or alternatively when non statutory services might better support a child or their family.
The main duties of the post holder will be to ensure you are focussed on the needs of children and ensuring they are addressed, minimising drift and checking the consistency of care planning and decision-making.
You should be a registered experienced Social Worker, have experience of working with Care and Support plans as well as Child Protection. You should also be able to demonstrate skills in chairing and reviewing care plans. An ability to work under pressure and to strict deadlines and timescales is important. Good communication and organisational skills are also vital for this role.

An exciting opportunity has arisen to join the management team with Cardiff’s Child Health & Disability Team due to development of the service.
Cardiff’s Disability Team is a multi-disciplinary team which consists of social workers, social work assistant, support planners, PBS trained workers and carers assessors. The team offers support, guidance and interventions for children, young people and their families who have a sever or profound disability and those who are experiencing mental health difficulties. The service is developing its offer to children with additional needs including those where emotional health and wellbeing is present this is an exciting time to join a progressive team where children’s needs are at the centre of our planning.
The team is led by a permanent Operational Manager who has experience of managing children with complex needs. The team is split into two teams and you will join another established team manager to complete the team.
Joining the team you will offer leadership to a team of health and social care professionals including, social workers, social work assistants, support planners and carers assessors. The Principal Social Worker will assist you in supervising staff and deputising.
The work is varied and challenging and encompasses work with children in need to those children in care and everything in between. You will work with children and young people who have complex and diverse needs including those children who are either in Tier 4 Mental Health provision or at risk of admission.
About you…….
• Ideally you will be a qualified professional within either a suitable health or social care degree with experience of leading a multi-disciplinary team

MAIN PURPOSE OF THE JOB:
Supporting tenants with their individual Person Centred Plan to achieve maximum independence.
KEY DUTIES AND RESPONSIBILITIES:
Support individuals needs and wishes as identified in their Person Centred Plan, this will involve helping tenants achieving objectives for themselves and maximising their independence.
Support tenants to prepare meals, undertake household tasks, shopping and self-care. To participate in leisure activities, social events and to escort to appointments e.g. doctors, dentist etc.
The support worker will be responsible for addressing any tasks e.g. cooking, laundry, sewing and cleaning that for any reason the tenants cannot undertake themselves, as identified in their Person Centred Plan.
Assist and support tenants with their medication inline with current policies and procedures.
Assist and support tenants to manage their finances on a daily basis
Undertake sleep in duties based on the needs of the network.
Observe and report any signs of illness or any other matter likely to effect the well-being of the individuals concerned.
Participate and contribute to staff meetings maintain and update records as required in organisational policies and procedures.
It is a requirement to work on a rota basis, which will include bank holidays and weekends; it is an expectation that employees are flexible in their approach and are prepared to cover colleagues absences.
It is an expectation that as part of the departments continuing development programme, you will from time to time be required to work in different locations within the network.
Undertake such other work as may be determined by the Strategic Director of Health and Community Directorate from time to time, up to or at a level consistent with the principal duties and responsibilities of the job.
On occasion, support tenants in attending medical appointments.
Experience
Experience in a Care setting of addressing Service Users personal needs in line with individual care plans.
Ability to maintain records as required verbal/written.
Skills & Abilities
An awareness of risk management.
Ability to effectively communicate with colleagues and Service Users.
Helping/encouraging clients to undertake physical care needs.
Supporting Service users to maintain social contacts.
An understanding of challenging behaviour in Care context.
Knowledge of community and social care resources.
An understanding of Equal Opportunities and related policy.
