About the role
Under the supervision of the Senior Practitioner, you will work with adults who may require social care services, and their family and carers, assessing their social care, functional and health needs and to plan, implement, review and monitor actions and outcomes.

This includes financial assessment within the framework of the relevant legislation, Care Act eligibility criteria, and safeguarding adults’ policies and at all times work towards giving a greater choice, independence and social inclusion for the service user

You will review and monitor each support plan and the quality of the service provision, making changes in response to evolving needs and service principles.

Modelling best practice and in line with legislation, policy and guidance, you will promote the wellbeing, dignity of service users enabling self-directed support and managing risk to ensure safety.

In line with policy and guidance you will provide recommendations including implementation of reablement or rehabilitation, use of Telecare, and/or equipment/adaptations to promote independence, and where required support implementation and monitoring of benefit to service users. You will consider manual handling and provide solutions for individuals and carers to work with.

You may be required to provide reports to the housing department on the functional impact of disability of individuals and recommend suitability and adaptability of properties.

You will work with housing and environmental colleagues to make recommendation for disabled facilities grants.

Job Purpose:

The core focus of this job is to:

  1. Undertake the responsibilities of the AMHP as determined by the Mental Health Act 1983 (as amended by the Mental Health Act 2007) and associated legislation, codes of practice and guidance.
  2. The post holder will be a member of an integrated Community Mental Health Team and will undertake duties required by that role
    OR ALTERNATIVELY,
    the post holder will be a member of a specialist team, eg assertive outreach, crisis resolution, Intensive Home Treatment Team, learning disability, CAMHS , EDT or adult’s social care team, and will undertake duties required by that role.
  3. Provide professional advice to colleagues, managers and partner agencies in relation to the application of mental health law.
    Qualifications and Training
  4. A professional Social Work qualification; an Occupational Therapy qualification; a Nursing qualification where field of practice is mental health or learning disabilities; Chartered Psychologist.
  5. Professional registration with the appropriate body. (HPC, GSCC etc)
  6. Approved Mental Health Professional Mental Health Act 1983.*
    *Prospective AMHP’s may be accepted in a number of roles subject to 1 and 2. above and supported to train to gain AMHP status.

You will work to coordinate the Discharge to Assess cases for Pathways One and Two. Under the direction of a Senior Practitioner you will work in a proactive way to collate information from a number of relevant sources to provide the People Division with all the information that they require to efficiently facilitate discharges from Pathway One and Two and to report on the status or all cases.

This could include (but not exclusive):
• Attending all the relevant Multi-disciplinary meetings at the relevant care homes so that accurate records are maintained.

• Report to Team Managers on progress with all elements of the process including allocations, assessments, and obtaining services
• To liaise with Sirona on the Delayed Transfers of Care. To check the data provided and to ensure accurate weekly reports.
• Act as a central point of contact for both Sirona and the Community Teams for information on the progress of Pathway One and Two cases.
• Monitor the progress of Pathway One and Two cases, and provide progress reports to the relevant managers.
• Regularly liaise with the brokerage team to provide information on the cases awaiting services and to ensure that they are treated as a priority.

This role will be integral to the efficient functioning of Discharge to Assess as it will be a central point of contact for the wide range of information required.

General description of role

To assist the Senior Engineer to deliver an efficient highways development control function. To help facilitate development whilst meeting the Council’s statutory obligations in regard to road safety, active travel and maintaining the free flow of traffic.   1. Provide pre-application advice on highway design issues to developers and their agents, to agree the scope of Transport Assessments and Transport Statements and request traffic surveys where necessary to help in the assessment of development proposals.
  2. Assess the geometric layout of highway design proposals submitted as part of planning applications in accordance with relevant design standards and guidance such as Manual for Streets; TAN 18 and the Design Manual for Roads and Bridges.

  4. Attend meetings with developers, the general public, elected members, town and community councils and other interested parties, as required, and represent the Highway Authority as necessary as part of the planning consultation process.
  5. Assess, give feedback, and provide final approval for detailed construction drawings and specifications provided by developers, and consultants acting on their behalf, for works covered by Section 38 and/or Section 278 agreements.
  6. Supervise construction works to ensure they are carried out in accordance with the approved design drawings and construction specifications as part of Section 38 and/or Section 278 agreements.
  7. Appraise Transport Assessments and Transport Statements submitted as part of planning applications to ensure that they provide a robust evaluation of the potential impact of development proposals.
  8. Attend the Council’s Planning Committee, when required, to answer any questions the Committee might have in regarding to the impact of a development on the highway network.
  9. Provide written professional evidence to support the Highway Authority at Planning Appeals and to attend Planning Appeals as necessary.
  10. Provide assistance to the Senior Engineer (Development Control) and to the Legal Department regarding any details required in the drafting of Section 38 and Section 278 agreements.
  11. Liaise with other internal teams to seek views on more specialised issues such as traffic signal design or public transport provision.
  12. Supervise tests required as part of both the design and construction process to ensure that they are carried out in accordance with standard industry processes. Examples of these tests include CBR tests and percolation tests.
  13. Prepare Traffic Regulation Orders in liaison with the Council’s Legal team, including the preparation of schedules and consultation with stakeholders in accordance with statutory requirements.
  14. Provide assistance to the Senior Engineer and Section Manager to identify and develop the transport requirements for strategic plans and regeneration proposals.
  15. Operate and maintain the Council’s transport planning software such as TRICS, TEMPRO and JUNCTIONS.
