Exciting opportunities have arisen for qualified Social Workers to join our award-winning Integrated Community Services (ICS) Team.
The service encourages a ‘Home First’ approach, ‘right care, right time, right place’, working closely with our in-house CQC Registered reablement service, START and partner agencies.
As a Social worker you will be actively involved in facilitating complex discharges from Acute and Community Hospitals within Shropshire and within neighbouring counties to repatriate Shropshire residents back home whilst also providing an Admission Avoidance service.

The role includes working under pressure at times of high demands from our system partners and having personal drive.
In collaboration with other professionals make assessment of individual need and implement and monitor the agreed support plan and keeping safe plan, as appropriate.
Consider and assess Mental Capacity and co-ordinate Best Interest decision making forums where appropriate. Documenting decisions and assessments accurately.
The ability to recognise where a Deprivation of Liberty or a restriction may be an issue. Liaising with registered providers when a referral to the DOL’s Team is appropriate.
To contribute to the development of all aspects of the team and the service provided.
To co-ordinate the use of all resources promoting the creative and flexible use of personal budgets to meet eligible needs and accessing community services and activities
To maintain accurate records with relevant, objective, detailed information and with accuracy in a timely manner and in accordance with departmental policies and procedures.
To attend and participate in team meetings, supervision and performance and appraisal and any quality assurance processes.
To take part in duty systems, as required.
To comply with safe systems of work in accordance with the Department’s Health and Safety and Welfare policies and procedures.

Successful candidates will be aligned to one of the five area teams - Central, North East, North West, South East, South West - but will be required to work flexibly across the county as and when service demand requires.

Information Officer

Main Purpose of Post:
The Registered Manager will set the operational direction and oversee the effective running of the service provision to meet the statement of purpose and children and young people’s needs identified within care and support plans. Golwg y Bannau provides placements to meet the needs of children and young people with complex needs and learning disabilities.
Principal Responsibilities:
- To manage the provision in order to ensure that the needs and outcomes for children and young people with disabilities and complex needs are achieved. To carry out such other duties, appropriate to the post, as may reasonably be required.
- To oversee all aspects of the day to day running of the home. This will include ensuring staffing rotas are maintained and that adequate staffing levels are in place, taking account of staff sickness and annual leave commitments. To participate in rota working arrangements where applicable with overall responsibility for managing the rota system.
- To lead on the recruitment, induction and training of staff to ensure that a sufficient number of suitably qualified, trained, skilled, competent and experienced staff are deployed to work within the Care Home.
- To ensure staff are fully aware of the policies, practices and procedures of the children’s home and provide high quality supervision and training for all staff to enable ongoing learning and development.
- To manage the Care Home’s budget with prudence and in keeping with Powys policies and procedures.
- To ensure the Care Home operates in accordance with international and national best practice and in keeping with all relevant legislation and regulation including the Social Services and Wellbeing (Wales) Act 2014 and Regulation and Inspection of Social Care (Wales) Act 2016. To keep up to date with new developments and legislation in childcare practice.
- To manage the behaviour of individual and groups of children and young people appropriately and within the guidelines laid down by the Care Inspectorate Wales.
- To ensure that all activities within the Home are carried out with due regard to health and safety requirements, undertaking risk assessments where appropriate and being responsible for the health, safety and welfare of all staff and young people.
- To actively promote, participate in and ensure the provision of high standards of physical care, hygiene, safety, and welfare of all young people. To ensure the provision of a warm accepting environment within the home whereby the emotional, social, educational, cultural, and spiritual needs of individual young people can be achieved, and in which teamwork amongst staff flourishes.
Registrations, Qualifications &Training:
⦁ To be registered with CIW as a Home Registered Manager or willing to register within specified time scale.
⦁ Level 5 Diploma in Leadership for Health and Social Care Services (Children and Young People’s Residential Management) Wales and Northern Island or a predecessor qualification laid down by Social Care Wales.
⦁ Proven commitment to ongoing professional development and training.
⦁ Additional qualification relating to Autism, learning disabilities or behavioural issues.
Knowledge:
⦁ Working knowledge of the current legislation, regulatory and policy framework relating to the registration and inspection of children’s homes in Wales. Regulation and Inspection of Social Care (Wales) Act 2016.

Key Tasks:
To ensure oversight of all care proceedings in Powys (or an area within Powys). Tasks:
⦁ Develop and maintain a database in order to monitor the progress of all care proceedings and pre-proceedings in the department/area
⦁ Provide reports in relation to this information to assist the Local Authority to project demand for placements, therapeutic or other relevant services.
⦁ Ensure that all children who are the subject of care proceedings have been subject to the relevant permanence planning processes in order to determine the care plan.
⦁ Ensure that Social Workers have the resources they need in order to submit court reports on time and to a high standard.
⦁ Provide practical assistance to Social Workers to ensure that they can complete court work to a good standard.
⦁ Ensure that all work to be filed before the court has been through a QA process and that the views and feelings of the child are reflected and have been actively considered in drawing up the statement.
⦁ Attend permanence planning and care planning meetings for children who are the subject of care proceedings.
⦁ Ensure that in relation to pre-proceedings, that the plan of improvement is relevant to the concerns about the children and that progress is made in a timely way, or to alert the Team Manager or Senior Manager if the progress of the family is drifting.
⦁ To attend Legal Planning Meetings in an advisory capacity.
⦁ At all times to work in co-operation with the Team Manager of the social work team in which the care proceedings are held.
⦁ To liaise with CAFCASS in relation to any concerns that they may have about the conduct of care proceedings in Powys.

Experience:

You will be working in partnership with social workers and team managers to support creativity, fulfilling practical roles at times and consultative positions at others, depending on the identified needs. There will be no case allocation. Full Terms of Reference for the Team available through discussion. You will be a motivational communicator with strong interpersonal skills, and model and lead a restorative and strengths-based approach to professional relationships and practice. You will work with an innovation project lead to build confidence in staff to work and think creatively and support practical and effective work to identify and achieve the best outcomes for children and young people at pace.
