JOB PURPOSE: The core focus of the Occupational Therapist role is to maximise the independence and safety of vulnerable people in North Yorkshire who have social care needs. The South Locality (Discharge Services) Team works with customers who are either in Salisbury District Hospital or in Discharge to Assess (D2A) placements located in the south of Wiltshire. The qualified Social Workers in the team carry out Care Act Assessments, assessments of mental capacity and best interest decision making, supervision of other staff and undertaking the role of enquiry officer for safeguarding adult s.42 enquiries. The team consists of a team manager, assistant team manager, advanced practitioner social worker, experienced social workers, newly qualified social workers and social care practitioners.
Face to face visits to carry out assessments are required. Time spent with case recording, typing up assessments, making calls and other aspects of care management can be carried out from the team's Salisbury Hospital office, Bourne Hill (Council offices in Salisbury city centre) or from home.
The recording and assessment system is Liquid Logic. Laptops and earphones are issued and use Microsoft services, including Microsoft Outlook and Microsoft Teams.
Indicative qualifications
